Individual Product Order Total Fee

This contribution borrows heavily from Individual Product Shipping Prices v1.0. I operate a rental site, and I needed a way to calculate the shipping insurance based on the value of each item, and not the order total (rental fee). This allows you to set a fee for each product that is added into the order total. I calculate the insurance cost for each product and assign it to each item. You can use this for insurance, special handling fees, etc.
